We study a liner shipping network design problem to optimize regular Energy efficiency is widely promoted as a primary objective in the design and operation of combined heat and power (CHP) systems. However, whether higher energy efficiency necessarily translates into lower operational costs remains insufficiently understood. This study revisits the relationship between energy efficiency and cost efficiency in CHP systems by jointly examining system design choices and operational decisions under realistic demand and price conditions. We develop an integrated optimization framework that captures the trade-offs among fuel consumption, heat–power coupling constraints, investment decisions, and time-varying electricity and heat demands. Our analysis reveals that maximizing energy efficiency does not always lead to cost-optimal outcomes, particularly in environments with volatile energy prices, asymmetric demand profiles, or binding operational constraints. In several cases, designs that sacrifice a limited amount of energy efficiency achieve substantially lower total costs through improved operational flexibility. Numerical experiments based on representative CHP configurations demonstrate the conditions under which energy-efficient designs become cost-inefficient and quantify the associated opportunity costs.
